Reregistering a bike from Non-Op status. What DMV form do I need?

So I went to the DMV and got the correct information. The (high) fee quote I got on the phone was not related to the vanity plate after all. The high cost was for two years of registration. The original registration due date is in 2 days and they don’t prorate it. Once I got to the dmv they told me to come back in two days and the fee will drop by half.

Glad you caught that. I just made the mistake of taking my bike out of non-op a few months before it's reg date. Had to pay for the whole year and then pay again to register it a month later. Really sucked.
