I only use it for "pinned the throttle" where I mean I've turned the throttle back so it can go no further.

^ That's pretty much my definition of pinned, too. I don't have to be in the highest gear. Like, on a super goaty road, pinned in fifth is pinned enough. Sixth gear would just be stupid (fun).
